| Medical Assurance Group recognizes that our clients
are seeking to ensure our organization is continually assessing our
compliance with the Health Insurance Portability and Accountability
Act (HIPAA). Our goal is to protect the privacy and security of individually
identifiable health information and our clients’ ability to
use our services.
A primary focus of our Privacy Officer is to research privacy and
security regulations and legislation on both the federal and state
level. We are continually assessing the impact of HIPAA on MAG’s
business processes. We remain confident that our efforts produce
the results necessary to achieve our compliance goals.

| Information Collection: |
MAG will collect customer identifiable information
to provide services or to inform you of services that may better
meet your needs. This means that MAG may use your customer identifiable
information to market new services to you that we think will be
of interest to you, but we will not disclose your customer identifiable
information to third parties who want to market products or services
to you.
MAG also obtains personal health information in connection with
specific customer requested services. We will not use or share personally
identifiable health information from any examinee, for any purpose,
other than providing all relevant information to the medical specialists
responsible for conducting requested service.

We maintain strict physical, electronic, and administrative
safeguards to protect individual and customer information from unauthorized
or inappropriate access. We restrict access to this information
to those MAG employees who need to know the information to respond
to your inquiry or request. Employees who misuse customer information
are subject to disciplinary action.

| When you access the Internet, the computers that host
Web sites you visit (Web servers) automatically receive some anonymous
information. This "usage data" may include a record of which
pages a Web browser has visited. MAG servers receive usage data when
customers visit the MAG Web site. MAG may use usage data to provide
advertising about services that may be of interest to MAG customers,
but MAG will not use information about your activities on the Internet
together with any information that identifies you without your consent.
Small files called "cookies" may be attached to your
Web browser. These files identify your browser and save information
such as passwords so that Web sites can recognize you. You can set
your browser to disable cookies, as our Web site does not use cookies,
but other Web sites may not work properly if you do this.
